Regular Physical Therapy Appointment
Please click and fill out the Patient Registration and Medical Information forms prior to your first visit at your scheduled center. Once you hit the send form button at the bottom of the form, your form will be electronically sent to the center where you are scheduled. You may also download and print out your forms below. Please remember to select the correct outcome form below to fill out. If you choose to print out your forms, please bring them to your first scheduled appointment. Please note, if you choose to fill out your paperwork at our center, you must arrive early as we do not allow for this within the scheduled appointment time.

Vestibular and Balance Physical Therapy Appointment
