FIRST VISIT
  • Be sure to bring loose-fitting comfortable attire (T-shirts, shorts, athletic shoes, etc.) to allow for evaluation and movement

  • Please arrive 20 minutes prior to your scheduled appointment

  • Remember any relevant insurance information with you for registration

  • Bring Patient Registration Form and Medical Information Form

  • Simply check in with our receptionist upon arrival



PAYMENT SERVICES
  • Most all major insurance providers are accepted

  • We will be happy to verify your insurance benefits prior
    to your first visit

  • Medicare – OPTC is a Certified Rehabilitation Agency

  • Billing is done internally at no cost to you

  • Our Operations Manager and/or the Billing Department will gladly address any questions you might have regarding billing procedures, or the status of your account


ESTABLISHING INDIVIDUALIZED GOALS

Before designing your individual program, your Oxford Physical Therapy Centers physical therapist will conduct a thorough evaluation, assess your condition, learn your goals, and discuss all findings with you.

Throughout your program, our expert staff will communicate with all those involved-you, your doctor, and other health care professionals to keep everyone current on your progress.

Since you are an integral part of reaching your goals, it is important to us that you feel informed. If you ever have any questions during or after your visit to Oxford Physical Therapy Centers, please be sure to ask.
