Due to Medicare guidelines those patients over age 65 or any patients that are Medicare eligible will need to be evaluated to determine medical necessity and if the club services are exempt from Medicare's skilled care guidelines prior to participation in the club. For these members an ABN form will also be required.
Flex spending/HSA dollars are eligible to be used for club fees.

Posture prevention program
Work with a PT to develop a protocol of exercises geared at prevention of pain and improvement in posture.
First 6 visits (4 months) using the band connect device to focus on maintaining good posture.
After you complete first 4 months of the Posture Power Pack membership- add 6 months of hybrid in person and/or virtual consultations with your Band Connect Device.
After you complete first 4 months of the Posture Power Pack membership- add 6 months of VIRTUAL ONLY consultations with your Band Connect Device.
Service Fee Disclosure: A non-refundable service fee of 3.5% will be charged on the transaction amount on card not present transactions. This includes mail-in, phone-in and online payments.
